I can't believe they aren't able to go through the literally millions of crossing scripts in a huge-ass game like this!

These games are usually programmed by different teams as separate modules, only bringing all together at the end. The scope makes it hard to use any other software engineering methods, so I'm more surprised that they're aren't more conflicts... but on the other hand, that's why you do lots of beta testing, preferably with gamers that aren't connected to the dev team.
