Madness war axes can be forged from madness ore by the smith Cutter who operates Cutter's Weapons in Crucible. She can forge one war axe from three pieces of madness ore, and if you have a war axe matrix, you will get an enchanted war axe.
The quality of the war axe and, if applicable, its enchantment is determined by your character's level when you ask Cutter to forge the war axe for you.
